
Description
Non-profit hospital and Health Departments are required, every three to five years, to conduct a Community Health Assessment (CHA) which uses a data-driven approach to determining the health status, behaviors, and needs of the population. This information is used to formulate a Community Health Improvement Plan (CHIP) utilizing strategies to improve health and quality of life. Mental health and substance misuse/abuse are commonly identified as priority health issues.
This course emphasizes the importance of these processes to reduce duplication of efforts by sharing and coordinating resources, moving towards an efficient, sustainable community approach to prevention.
