Non-profit hospital and Health Departments are required, every three to five years, to conduct a Community Health Assessment (CHA) which uses a data-driven approach to determining the health status, behaviors, and needs of the population. This information is used to formulate a Community Health Improvement Plan (CHIP) utilizing strategies to improve health and quality of life. Mental health and substance misuse/abuse are commonly identified as priority health issues. This course emphasizes the importance of these processes to reduce duplication of efforts by sharing and coordinating resources, moving towards an efficient, sustainable community approach to prevention. This course takes the learner through a surface-level view of four large surveys conducted in Montana to identify the history, collection methods, report types, and utilization of the survey results in prevention.
